La celiaquía es una enfermedad autoinmune que afecta a muchas partes del cuerpo. Aunque la afectación es mayoritariamente intestinal también existen otros síntomas extraintestinales y enfermedades asociadas a ella.
Es por eso que cuesta mucho de diagnosticar esta enfermedad, ya que los síntomas son múltiples y afectan de manera diferente a cada persona que la padece. Actualmente, se dice que el 75% de la población que es celiaca no está diagnosticada. Por cada caso diagnosticado hay entre 5 y 10 sin diagnosticar.
Si habéis pasado por una situación de diagnostico tardío, os podéis imaginar lo mal que lo debe pasar la gente que aun no le han hecho las pruebas pertinentes.
Es por eso que es tan importante seguir las pruebas diagnósticas para la celiaquía al pie de la letra y no desistir si alguna sale negativa. Existe una «regla» en este caso y es que para que sea celiaquía deben dar 4 de las 5 pruebas positivas.

1. Sintomatología positiva
Lo más fácil de identificar es la celiaquía denominada clásica. Esto es porque los síntomas son los más típicos, es decir diarrea, vómitos, falta de crecimiento en el caso de la infancia, etc. Sin embargo, no todas las personas tienen los mismo síntomas ni se manifiesta la enfermedad de la misma manera. Por eso es necesario conocer que hay algunas enfermedades que son muy características de la celiaquía pero que no están relacionadas con el sistema digestivo. Algunas de ellas son:
- Dermatitis herpetiforme
- Anemia ferropénica
- Depresión
- Transtornos de la fertilidad (tanto en hombres como mujeres)
- etc.
También es necesario conocer que hay algunos grupos de personas que es más probable que sufran la enfermedad y que solo por el hecho de pertenecer al grupo de riesgo deberían hacerse las pruebas diagnósticas siguientes para incentivar un diagnóstico precoz.
Todas estas personas son susceptibles a padecer la enfermedad celiaca y es por eso que es la primera que se debe cumplir para que se puedan seguir haciendo más pruebas.
2. Anticuerpos: marcadores serológicos
La segunda de las pruebas diagnósticas de la celiaquía es la serología. Se trata de una analítica específica en la que deben comprobar si existen unos anticuerpos (AC) que son específicos de la enfermedad y la cantidad que existe de ellos. A continuación os dejamos cuales son y algunas características. Aunque no hace falta conocerlo porque el especialista debe comentártelo, nunca está de más.
- AC antigliadina (AGA): Son poco específicos para el diagnóstico pero en menores de dos años pueden ser los únicos AC que se encuentren. Eso sería válido para probar un control de dieta sin gluten.
- AC antiendomisio (EMA): Son muy específicos, por lo que hay muy poco falsos positivos. Sin embargo, no son muy sensibles, por lo que puede dar lugar a falsos negativos.
- AC antitransglutaminasa tisular (TTG): Es muy sensible, por lo que no hay mucho falsos negativos pero da lugar a falsos positivos. Estos son los más útiles para el cribado de la enfermedad celiaca, ya que existe una relación proporcional entre la cantidad de anticuerpos y el grado de lesión del intestino.
- AC antipéptidos deamidados de gliadina (DGP): es interesante de estudiar si sale positivo en niños menores de 2 años.
Por tanto, como hemos visto la serología negatia no excluye el diagnóstico pero una gran cantidad de AC si puede ser una evidencia del diagnóstico de celiaquía.
3. Pruebas genéticas
Esta es una de las pruebas diagnósticas para la celiaquía más determinante. Las personas celiacas deben tener una serie de marcadores genéticos del denominado sistema HLA para poder ser diagnosticadas. Aunque no todo el mundo que tiene estos genes tiene porque ser celiaco, también influyen los factores ambientales.
Es por eso que la genética es una condición necesaria pero no suficiente para que una persona sea diagnosticada. También existen algunos casos en los que no hay estos marcadores pero existe la enfermedad (son excepcionales).
Los que tienen que estar presentes son:
- HLA-DQ2: El 90-95% de la población celiaca tiene este marcador genético.
- HLA-DQ8: La mayoría de personas celiacas que no presentan HLA-DQ2, tienen HLA-DQ8.
4. Biopsia
Si todas las anteriores pruebas dan positivas se realiza la biopsia. Esta prueba diagnóstica es necesaria para evaluar el grado de lesión intestinal que está provocando la celiaquía. Si está presente la enfermedad (sobre todo si hay un diagnóstico tardío) es probable que las vellosidades intestinales estén afectadas.
Las vellosidades intestinales son unos pliegues del intestino delgado que se encargan de la absorción de los nutrientes que ingerimos. La celiaquía provoca que se atrofien y dejen de hacer su función. Si no desarrollan la absorción, es probable queesto provoque algunos síntomas como: desnutrición, amenorrea, talla baja, etc. Creando déficits alimentarios.
La gravedad de la lesión intestinal se mide en esta prueba diagnóstica mediante la escala de Marshal. Que va del 1 (un aumento del número de limfócitos intraepitelial) al 3C (atrofia vellositaria total).
Esta clasificación no determina grados de la celiaquía, solo el grado de atrofia, ya que no existen los grados de celiaquía. Esta se debería ir regenerando con la dieta sin gluten estricta.
5. Reacción a dieta sin gluten
Por último, si todas las pruebas diagnósticas han dado positivas y hay atrofia vellositaria es necesario hacer esta última prueba diagnóstica.
En este caso se trata de ver si el cuerpo reacciona de manera positiva a la eliminación del gluten de la dieta (por eso es tan necesario seguir consumiendo gluten antes de este paso, para que no haya alteraciones en las pruebas y se pueda ver la mejoría en esta).
Si no hay ninguna otra alteración que lo impida, los síntomas deberían empezar a remitir a las dos semanas. La normalización de la serología entre los 6 y los 12 meses y la recuperación intestinal a partir de los dos años, siendo mucho más rápida la recuperación en niños que en adultos.
Por eso también es necesaria una dieta sin gluten estricta, permanente y equilibrada para poder notar los cambios una vez se empiece.
De estas 5 pruebas, como decíamos antes, 4 deben ser positivas. Es probable que los anticuerpos no se vean reflejados en la sangre pero si el resto de pruebas si lo son, entonces puede haber diagnóstico.
